Once you submit your form, I will update your registration and insurance information and verify your coverage. Please remember that I require a minimum of 2 business days to verify coverage.

Client Portal

You must be a registered and current client to access the client portal. Please remember to keep your username and password in a safe and secure place. If you have trouble logging in, please email me at allyj@ootbc.net

To better serve you, Out of The Box Counseling has implemented an Electronic Health record (EHR) with a secure Client Portal. I have chosen to use a certified interoperable electronic health record that is in compliance with Florida state law.

It is a requirement that all current clients go through the process of creating their online Client Portal within my new system.

The Client Portal is a secure account that will allow you to:

• Update your contact information
• Complete and submit paperwork online before appointments
• View invoices, superbills, and billing documents
• Upload identification documents
• Access your telehealth link to conduct our sessions through videoconferencing
• View/cancel upcoming appointments
• Securely message your therapist

To initiate your portal access, I will send a link to your email address provided, with instructions on how to set up your portal account.
